Forms for Individual Online Counselling

Thank you for engaging the services of Clinton Power + Associates.

This page is for individuals who have a confirmed appointment and are preparing for their first online counselling session.

Required Forms

Please download and complete these documents and email them to [email protected] the day before your scheduled appointment:

1. Confidential Client Information – please complete details and sign.
2. Terms of Service Agreement – please read and sign. Leave the ‘enrolment’ section blank and you can decide if you want further sessions at the end of the first appointment.
3. Ezidebit – this is the system we use for payment. Please complete and sign. Leave the frequency section blank and you can decide if you want any further sessions at the end of the first appointment.

Please note, Clinton is unable to proceed with the session if we have not received your paperwork before your scheduled online meeting.
